**Qualifications**

**Education and Experience**

+ MD or DO degree.

+ Successful completion of a residency program.

**Licensure**

+ AZ MD License

+ AZ licensed physician Anesthesiologist

+ Board qualifed or certified

+ Credentialed SJHMC.

+ Membership in appropriate professional associations and societies

**Pay Range**

$80.00 - $750.25 /hour
