**Responsibilities**
**Position Summary:**
The Social Worker MSW performs psychosocial assessments and care planning as part of an interdisciplinary group while demonstrating competency in patient care. The Social Worker MSW assesses the practical needs of patients/families and refers patients to appropriate resources. Specializing in assisting with the completion of necessary documents and applications to access recommended resources and services. This position requires reliable transportation.
**Qualifications**
**Minimum Qualifications:**
+ Master degree in Social Work from a university or college accredited by the Council of Social Work Education required.
+ Minimum one year as MSW
+ BLS through American Heart Association
+ CA Drivers License, Auto insurance
+ Verbal and written communication and computer skills required
**Preferred Qualifications:**
+ Previous hospice/home health experience, or a combination of hospice and other related clinical experience preferred

**Pay Range**
$46.52 - $60.56 /hour
